What To Expect During Your First Visit

We provide a complete exam with same day results for our new patients. Once you have made your initial appointment, please provide any prior results/records as well as any other results from testing you have completed with your previous clinics. These are very informative in most circumstances and avoid any duplicate diagnostic recommendations by the doctor.

We're Here To Help, Our Consultation Process

A doctor on our team will then review any previously obtained diagnostics provided and perform an examination. Depending on the patient’s chief issue, the evaluation will include an exam and, if need, an eye dilation. An Rx for contacts or glasses, plan of care and treatment (if a serious issue is seen) options will be determined and discussed based upon your evaluation findings and any imaging reviewed. 

New Patient Forms

All new patients will be asked to complete patient information forms. These forms may be downloaded and completed prior to your arrival. These forms allow the doctor to provide a more thorough evaluation by understanding your history and current symptoms and also include our consent to treat, our authorizations to obtain your prior treatment records if necessary, insurance authorization and financial obligation.

Once these documents have been completed, a member of our staff will room you and complete your intake process.
